I am reporting a
A workplace incident might include a difficult interaction with another staff member or patient, a staff member or patient experiencing a physical or psychological injury (e.g. slip and fall), or a process that has not worked properly (e.g. patient booked unable to book correct appointment and becoming aggressive). A near miss clinical incident is when an error or accident occurs but there has not been any harm caused either by timely intervention or chance. An adverse clinical event is where there has been a negative outcome that was unintended, unexpected or unplanned. E.g. patient given a medication that they are allergic to, and suffering a reaction. A data privacy breach is when patient data is shared or viewed by others without the patient's consent. E.g. a referral letter is sent to the wrong person. An observation is for reporting items of a general nature that would benefit from review. E.g. that a soap dispenser is dropping soap onto the floor and creating a slipping hazard.
